In June 1997 a supply rocket ship collided with the Russian space station MIR. The collision resulted in the near loss of the three cosmonauts, and structural damage to the space station.

This presentation examines the multiple engineering, managerial and personnel causes that led to the collision, and why previous deficiencies in the MIR operation were not addressed.
